When visiting our offices for an appointment:

The information below and on the nav bar will help you if you have an appointment or are preparing for surgery.

If you have an appointment, please read below. If you are preparing for surgery,
please click on the link to the left.

PLEASE bring your X-rays and CT scans. These studies are extremely important. We cannot make surgical decisions without them.

If you have to make a choice, the CT scans are more useful to us than the chest X-rays.

If you have a new spot on your lung, a chest X-ray that is at least one year old is
helpful.

You should be able to check your X-rays or scans out, or pick them up, at the
facility in which they were made. A phone call ahead of your visit will help them.

Bring your insurance card.

Bring a list of your medications and doses, or bring your medicine bottles.

Be prepared to fill out a medical history when you arrive at our office, or download the form from this site, and bring it with you.

Bring the addresses and phone numbers of your personal physicians.

You may wish to call ahead of your appointment to make sure that the physicians are not delayed by surgery or emergencies.
